OBJECTIVE: Transsphenoidal surgery (TSS) is considered the treatment of choice in most patients with growth hormone (GH)-secreting pituitary adenomas. Several preoperative factors have been studied to predict postsurgical remission. Our objective was to design a score that could be used in the preoperative setting to identify patients that will achieve long-term biochemical remission after TSS. METHODS: A retrospective analysis of consecutive patients with GH-secreting pituitary adenomas that underwent TSS in our institution from 2000 to 2015 who fulfilled prespecified criteria were included. Logistic regression methods were used to evaluate independent preoperative variables predicting long-term remission. Beta coefficients were used to create a scoring system for clinical practice. RESULTS: Sixty-eight patients were included, with a mean follow-up time of 87 months. Twenty (29%) patients had tumors with a Knosp grade ≥ 3A. Gross-total resection was achieved in 43 (63%) patients. Thirty-three (48%) patients had long-term biochemical remission after TSS. In a multivariate analysis, the following variables were statistically significantly associated with long-term biochemical remission: age, adenoma size (diameter), Knosp grade, GH level, and insulin growth-factor 1index 1 at diagnosis. A score of <3 out of 8 total points was identified as a cutoff associated with long-term remission, with a sensitivity of 91.4% and specificity of 72.7% (AUC 0.867, OR 28.44, 95% CI 6.94-116.47, P = < 0.001). CONCLUSIONS: A novel, simple, easy-to-use scoring system was created to identify patients with the highest chances of long-term biochemical remission following TSS. This scale should be prospectively validated in a multicenter study before widespread adoption.

PURPOSE/AIM OF THE STUDY: To identify the inflammation indexes associated with the severity and functional prognosis in ischemic stroke. MATERIAL AND METHODS: A prospective study was conducted with ischemic stroke cases included in the i-ReNe clinical registry. Patients were divided into groups according to the severity on admission measured by the National Institutes of Health Stroke Scale (NIHSS) and the functional prognosis at 30 and 90 days of discharge measured by the modified Rankin Scale (mRS). RESULTS: We included 145 patients with a mean age of 61.5 ± 12.75, 97 (66.9%) were men. The leukocyte and neutrophil counts, Neutrophil-to-Lymphocyte ratio (NLR), Derived Neutrophil-to-Lymphocyte ratio (dNLR), Platelet-to-Lymphocyte ratio (PLR), Segmented Neutrophil-to-Monocyte ratio (SeMo ratio), and Systemic Immune-inflammation index (SII) were higher in moderate-to-severe stroke (NIHSS ≥6). NLR, PLR, SeMo ratio, and SII were higher in the group with severe disability and death at 30 days (mRS ≥4). In the multiple logistic regression analyses, SeMo ratio >14.966 and SII >623.723 were associated with moderate-to-severe stroke (NIHSS ≥6). In addition, SeMo ratio >7.845 was associated with severe disability and death at 30 days (mRS ≥4). CONCLUSIONS: Systemic inflammation indexes could be rapid and low-cost markers used in the initial evaluation of ischemic stroke, whose values could help to stratify patients according to their severity and functional prognosis. This is the first study to establish a relationship between ischemic stroke and the SeMo ratio.

OBJECTIVE: The authors report a single-surgeon experience with anterior clinoid meningiomas (ACMs) and propose a novel scoring system to predict visual outcomes based on preoperative risk factors. METHODS: A cohort study of all ACMs that were surgically treated by a single surgeon between 2003 and 2021 was performed. Visual function was assessed by an ophthalmologist pre- and postoperatively. Based on the combination of visual fields and visual acuity, 4 visual grades were described. Favorable visual outcomes were defined as mild visual deficit or intact vision postoperatively. Unfavorable visual outcomes were defined as a severe or moderate visual deficit. Predictors of unfavorable visual outcomes were identified using multivariable logistic regression analysis. A scoring system was then created using the resulting ß coefficient. A receiver operating characteristic curve analysis was performed to identify a cutoff point on the grading score for stratifying patients at risk for unfavorable visual outcomes. RESULTS: Fifty-two patients met all inclusion criteria. Twenty-five (48%) patients presented with intact vision, and 27 (51%) presented with some visual dysfunction. Postoperative favorable visual outcomes were achieved in 39 patients (75%). Among the 27 patients presenting with visual dysfunction, 14 (52%) experienced improvement after surgery. No new visual deficits were observed among the 25 patients with intact vision at baseline. Nine patients (17%) had a reversible complication. Multivariable analysis showed that severe preoperative visual deficit (OR 13.03, 95% CI 2.64-64.39; p = 0.002), radiographic evidence of optic nerve (ON) encasement (OR 4.20, 95% CI 1.06-16.61; p = 0.04), intraoperative evidence of ON invasion (OR 17.31, 95% CI 2.91-102.86; p = 0.002), an average ganglion cell layer thickness of ≤ 70 µm (OR 21.54, 95% CI 2.94-159.04; p = 0.003), and an average retinal nerve fiber layer thickness of ≤ 80 µm (OR 13.68, 95% CI 1.91-98.00; p = 0.009) were associated with unfavorable visual outcome. The predictive score included the following factors: abnormal optical coherence tomography (OCT) findings, radiographic evidence of ON encasement by the tumor, and severe preoperative visual deficit. A score ≥ 4 of 6 points was demonstrated to be the cutoff associated with unfavorable visual outcome, with a sensitivity of 80%, specificity of 88%, positive predictive value of 80%, negative predictive value of 88%, and area under the curve of 0.847 (95% CI 0.674-1.0; p = 0.003). CONCLUSIONS: The authors have designed a practical and novel scoring system to predict visual outcomes in patients with ACMs. This scoring system may guide preoperative discussions with patients and timely surgical intervention to yield optimal visual function outcomes. Although most patients have excellent neurosurgical outcomes, severe baseline visual deficits, ON encasement, and characteristic OCT abnormalities are associated with unfavorable visual function after ACM resection.

Introducción: La historia de los cuidados paliativos es extensa, pues se remonta a los hospicios medievales de la Europa antigua, para posteriormente centrarse en su desarrollo como lugares de acompañamiento para los enfermos en la etapa final de la vida; son considerados como un derecho y parte fundamental de la salud al estar entrelazados con los conceptos de dignidad humana, los principios de autonomía humana y de calidad de vida. Objetivo: Se buscó la realización de un estudio relativo al conocimiento de la disciplina de Cuidados Paliativos por parte de los estudiantes de la carrera de médico cirujano y partero de la Facultad de Medicina de la Universidad Autónoma de Nuevo León, además de la percepción hacia esta disciplina. Metodología: Estudio del tipo transversal y descriptivo, utilizando un cuestionario a partir del Palliative Care Knowledge Test (Prueba de Conocimiento de Cuidados Paliativos, PCKT) distribuido por medios electrónicos, requiriendo del consentimiento informado de los estudiantes para ser incluidos; aquellos que cumplieron con los criterios de inclusión fueron divididos en 3 grupos. Resultados: Se incluyeron 201 estudiantes; 115 se encontraban en el grupo de 10.o > semestre, 26 en el grupo de 1-5.o y el resto en el de 6-9.o; 84 habían recibido alguna formación en cuidados paliativos como materia optativa al no encontrarse dentro del marco curricular. Los temas de farmacología fueron los más desconocidos para los estudiantes de los 3 grupos, además de existir inexperiencia sobre cuándo debe iniciarse este tipo de cuidados. Conclusión: Se encontró la necesidad de incluir los cuidados paliativos dentro de las materias de tipo curricular para la adquisición de competencias de la disciplina como las relativas a dar malas noticias y cuidados al final de la vida, con el objetivo de establecer mejores áreas de preparación para los futuros profesionales médicos. Introduction: The history of palliative care is extensive, dating back to the medieval hospices of ancient Europe, to later focus on its development as a place of accompaniment for the sick at the end of life; it is considered a right and a fundamental part of health as it is intertwined with the concepts of human dignity, the principles of human autonomy and quality of life. Objective: The aim was to carry out a study on the knowledge of the discipline of Palliative Care by students of the “médico cirujano y partero” career of the Medicine Faculty from the Universidad Autónoma de Nuevo León, in addition to the perception towards this discipline. Methodology: Cross-sectional and descriptive study, using a questionnaire based on the Palliative Care Knowledge Test (PCKT) distributed electronically, requiring the informed consent of the students to be included; those meeting the inclusion criteria were divided into 3 groups. Results: A total of 201 students were included; 115 were in the 10th > semester group, 26 in the 1st-5th group and the rest in 6th-9th; 84 had received some training in palliative care as an elective subject as it was not within the curricular framework. Pharmacology topics were the most unfamiliar to the students in the 3 groups, in addition to inexperience about when this type of care should be initiated. Conclusion: It was found necessary to include palliative care within the curricular subjects for the acquisition of competencies of the discipline such as those related to giving bad news and end-of-life care; with the aim of establishing better areas of preparation for future medical professionals. INTRODUCTION: Obesity is considered an epidemic condition. It is related to the increased prevalence of chronic degenerative diseases, but there are no studies in the Mexican paediatric population on its relationship with sleep disorders. AIM OF THE STUDY: We sought to determine the prevalence of sleep disorders in paediatrics, as well as factors for their development. MATERIAL AND METHODS: An observational, analytical, retrospective study, was performed including patients aged 2 to 16 years, between March 2019-2021. The Paediatric Sleep Questionnaire was used to assess the presence of sleep disorders. RESULTS: Sixty-four patients were included, 27 (42.18%) of whom had sleep disorder criteria. Among these, obesity, i.e. weight percentile ≥ 75 and body mass index percentile ≥ 93.5, was a risk factor. Normal weight was protective. CONCLUSIONS: Obesity in paediatric patients may favour the appearance of sleep disorders; screening could favour early diagnosis, correct treatment, and better life quality.

Due to these psychological changes resulting from the stress produced by the disease, we sought to investigate the psychological impact of the pandemic on the university student community. MATERIAL AND METHODS: 1,283 students were surveyed, of which 1,149 students were selected. The majority of the subjects were female, and the overall average age was of 20 years. They were provided with an 82-question online questionnaire divided into four sections; looking for the prevalence of significant symptomatology of major depression and generalised anxiety using the PHQ-9 and GAD-7 scales; and factors that potentially affect the mental health of our university population. RESULTS: We found a high prevalence of significant depression (47.08%) and anxiety (27.06%) symptomatology, considering a score of 10 or more as cut-off point. There was no significant difference in depression and anxiety symptomatology between the health-care students and non-health-care students. CONCLUSIONS: Our results, together with what is observed in the literature, allow us to conclude that the college student population has a high risk of mental illness, and these should be taken into consideration for the search of effective strategies for detection and control of mental health illnesses. Undoubtedly, the COVID-19 pandemic is a red flag that shows the need to upgrade mental health programmes in universities and to validate virtual instruments.

Debido a estos cambios psi cológicos derivados del estrés que produce la enfermedad, se buscó investigar el impacto psicológico de la pandemia en la comunidad estudiantil universitaria. Material y métodos: Se encuestó a 1.283 estudiantes, de los que se seleccionó a 1.149. La mayoría eran mujeres y la media de edad fue 20 anos. Se les proporcionó un cuestionario en línea de 82 preguntas divididas en 4 secciones para indagar la prevalencia de síntomas signi ficativos de depresión mayor y ansiedad generalizada utilizando las escalas PHQ-9 y GAD-7, además factores que pueden afectar a la salud mental de nuestra población universitaria. Resultados: Se halló una alta prevalencia de síntomas significativos de depresión (47,08%) y ansiedad (27,06%), considerando como umbral un total de 10 o más puntos. No hubo difer encias significativas entre los estudiantes relacionados con la salud y los no relacionados con la salud. Conclusiones: Nuestros resultados, en conjunto con lo observado en la literatura, muestran que la comunidad universitaria es una población en riesgo de enfermedades mentales, por lo que el Gobierno debería considerar la creación de estrategias efectivas para la detección de problemas de salud mental; sin duda alguna, la actual pandemia de COVID-19 es un foco rojo sobre las necesidad de mejores programas de salud mental en universidades y validación de instrumentos virtuales.

BACKGROUND AND OBJECTIVES: Pediatric cerebrovascular disease carries significant morbidity and mortality. Early recognition of a pediatric stroke as well and its most common risk factors are important, but that diagnosis is often delayed. It is believed that the incidence in our center is higher than it appears. This study aims to assess the incidence and characteristics of the pediatric stroke in our university hospital. Likewise, this study seeks to evaluate if a longer symptoms-to-diagnosis time is associated with mortality in patients with ischemic stroke. METHODS: A retrospective study including children with stroke admitted to the UANL University Hospital from January 2013 to December 2016. RESULTS: A total of 41 patients and 46 stroke episodes were admitted. About 45.7% had an ischemic stroke and 54.3% had a hemorrhagic stroke. A mortality of 24.4% and a morbidity of 60.9% were recorded. Regarding ischemic and hemorrhagic stroke, an increased symptoms-to-diagnosis time and a higher mortality were obtained with a relative risk of 2.667 (95% confidence interval [CI]: 1.09-6.524, p = 0.013) and 8.0 (95% CI: 2.18-29.24, p = < 0.0001), respectively. A continuous increase in the incidence rate, ranging from 4.57 to 13.21 per 1,000 admissions comparing the first period (2013) versus the last period (2016), p = 0.02, was found in our center. CONCLUSIONS: Pediatric stroke is a rare disease; however, its incidence shows a continuous increase. More awareness toward pediatric stroke is needed.

Congenital medulloblastoma is a rare brain tumor that appears in less than 1% of pediatric patients. Congenital medulloblastoma has a poor prognosis and should be suspected in patients with clinical manifestations of hyporeactivity, slow suction reflexes, and the presence of hydrocephalus. Herein we present the case of a 12-day-old female newborn who developed non-communicative hydrocephalus, hyporeactivity, and hyporeflexia. Magnetic resonance imaging of her brain showed a heterogeneous and cystic mass on the posterior cranial fossa. A suboccipital craniotomy was performed. The histopathologic analysis reported a congenital medulloblastoma. She remained in hospital until her death at 112 days old. This is one of the first case reports with clinical-radiological and pathological documentation. Awareness of this diagnosis can allow prenatal intervention, rendering a better prognosis. This case report exemplifies the importance of good prenatal follow-up.

With the emergence of effective vaccines against SARS-CoV-2, the need to find the determinants for its correct distribution is created, with health personnel being an essential group where coverage must be ensured. MATERIALS AND METHODS: A cross-sectional study was conducted through an electronic survey that was distributed to health personnel in the state of Nuevo Leon, Mexico. Potential determinants for vaccine hesitancy were covered including their intention to be vaccinated against SARS-CoV-2. RESULTS: A total of 543 responses were collected from health professionals, which 30 (5.5%) of the subjects stated that they would reject SARS-CoV-2 vaccination. How well informed the subjects were was the most determining factor, where misinformation related to vaccination and COVID-19 was the most important cause of vaccine rejection. CONCLUSION: The implementation of educational strategies for health personnel are required.

Material y métodos: Se encuestó a 1.283 estudiantes, de los que se seleccionó a 1.149. La mayoría eran mujeres y la media de edad fue 20 años. Se les proporcionó un cuestionario en línea de 82 preguntas divididas en 4 secciones para indagar la prevalencia de síntomas significativos de depresión mayor y ansiedad generalizada utilizando las escalas PHQ-9 y GAD-7, además factores que pueden afectar a la salud mental de nuestra población universitaria. Resultados: Se halló una alta prevalencia de síntomas significativos de depresión (47,08%) y ansiedad (27,06%), considerando como umbral un total de 10 o más puntos. No hubo diferencias significativas entre los estudiantes relacionados con la salud y los no relacionados con la salud. Conclusiones: Nuestros resultados, en conjunto con lo observado en la literatura, muestran que la comunidad universitaria es una población en riesgo de enfermedades mentales, por lo que el Gobierno debería considerar la creación de estrategias efectivas para la detección de problemas de salud mental; sin duda alguna, la actual pandemia de COVID-19 es un foco rojo sobre las necesidad de mejores programas de salud mental en universidades y validación de instrumentos virtuales.
